Dermatitis herpetiformis: pathogenesis, diagnosis and treatment
Dermatitis herpetiformis (DH) is a chronic autoimmune blistering skin disease characterized by granular IgA precipitates in the papillary dermis and by an association with celiac disease. Epidermal transglutaminase and tissue transglutaminase are considered to be autoantigens in DH. Serum levels of IgA antibodies to both transglutaminases correlate with dietary gluten intake and probably with parameters of disease activity. A gluten-free diet is the treatment of choice for patients with DH. Dapsone should be use during the period until the gluten-free diet is effective.
